How do I get Custom Cursors?

I want to be able to use a custom image as a cursor in my game, but can't for the life of me work out how.
This has been bugging me for a while, and I'd appreciate any help.

But i this is a software cursor. As long as i know a hardware cursor is impossible!?

It's a hardware cursor in the sense that it's rendered in a hardware-accelerated manner.
